Help, Equipment list Software/Form?

Dear List,

I am trying to find a way to get an equipment list form that I can email to assistants so that they know what to get from my storage space. Right now i have a form that I write on, like (5) matthews stands, (2) Med softboxes, (2) packs, (5) strobe heads. Ect, with a notes section at the bottom. I was wondering if anyone knows where I can get a program where I can customize the thing (med softbox) and it has a space next to it where I can put in a number so the assistant knows what to put on the list. I am just trying to get away from faxing the form all the time, not everyone has a fax machine. I would rather email a pdf or something. I would appreciate any help.

My simplest and cheapest suggestion:

Just set up a text file template.
Write your standard list in Text Edit for OS X or SimpleText for OS 9.
Save the file, say StandardList.txt.

Open the file and change the numbers and add the custom instructions
and then copy and paste into the body of an email.

Also, you can send text files as attachments.

If you want pdfs, there is Acrobat, or Photoshop can be a word processor,
or In Design can save pdfs.
There are probably cheaper ways to create a pdfs.
Also, OS X has screen shot saving options, using Services->Grab.

But, why make it complicated if you don't need to.

You can also build it as a spread sheet in Appleworks. This is how I have made my custom invoices. The headings and catagories are all saved, and I can merely add quantities, and it can create totals if I wish. Also, it does not have to look "spreadsheety". I do not have any grid or cell-like lines showing. I looks like a clean document. I also added a header with my logo and a footer with contact info.

If I want to email one I have filled out, I save it as a PDF and the recipient get this really neat looking professional document.
