Also, make the form part of a data base, and give each one a serial number. That way you can make them searchable, and you can use that serial number to archive the job.
Right now I'm using it when people come into my studio for passport photos. It gives them a little something to do while I'm processing and printing their images.

Ultimately my purpose for having people fill the form out is to add them to my contact list. I constantly want to build the network of people that I can inform about new promotions, new products and services etc. A secondary purpose is to occupy people's time while they are waiting for me to process and print out their passport photos.

How did you hear about us....NOT important. I mean, do you really care? They are going to tell you. And if you really want to know, ask them when you are photographing them.
Do you want to receive updates and discounts, whatever that says...OF COURSE they do! Who wouldn't. Don't ask! Send them! If they don't want them, they'll tell you.
What I WOULD ask, their birthday, their anniversary, their hobbies, their favorite teams, etc.
Every birthday or anniversary I send a card to my clients. Handwritten. Often times, I'll give them a phone call. Everyone enjoys a birthday wish.
Hobbies, etc. these are going to help you with photography ideas, poses and props that create images that SELL!!
