many clients won't say where they heard of you, or give a general answer (internet). It's essential to know what advertising is working and what is not.
I've gotten a lot of calls from HS seniors lately - two things are working for me (posters and a rep). Which is driving the calls? I need to know- the posters cost me $25, the rep $250, so far. Based on geographical spread and no one mentioning the rep, it's the posters. Knowing this means I will flood the area with posters and quit having senior reps, or drastically cut back on what they get for what they give me.

Depends on the client what I ask - name, address, email and where you heard of me for everyone. For seniors and brides they fill out a form - kind of a survey. Questions vary by client, but for seniors - where do they hang out, what movie theatre do the frequent, hobbies/sports/activities, recomend a friend, type of music they listen to (so I play that during their session, can target my ads perhaps in the future). For brides - where did they buy their wedding dress, flowers, etc so that I can send their vendor pics after the wedding of the dress or flowers or cake, etc.

For events I have a one liner that asks if they'd like more info on wedding/senior/baby/family portraiture.

At bridal shows I've handed out surveys - brides love them and it gives you a chance to chat with them a bit - and learn your market, price, etc.

I like the one about asking where they got the dress, etc. and then send pics to the vendors. I typically do this on location, bridal portraits or something, just in conversation. It's something to talk about and creates a conversation, thus relaxing the bride and building a relationship. Makes it more personal.
